The present building was built in 1334, together with San Lorenzo and Santa Maria del Carmine churches, upon a pre existing church dedicated to Santa Maria di Betlemme (1180).
The façade is the result of a neogothic style restoration carried out in the 20ies by the architect Camillo Guidotti and the belfry was raised in 1938 by the architect Pietro Berzolla.
The interior has a so called "hall" structure (naves have the maximum height), a structure imported from the Germanun Westfalia region. The pictorial decoration is the result of a restoration work. The only original element is a representation of San Rocco (end of 15th century approximately) who, according to tradition, stopped at the monastery of Sant'Anna during his pilgrimage. The iconography of the painting "Madonna in labour" is very peculiar (it's been attributed to Antonio de Carro and it probably dates back to the first half of the 15th century.
In the vault of the central span there is the representation of a "Resurrection" frescoed by Bernardino Gatti known as Soiaro (1543-1545).
